Prospective clinical validation of the Eleveld propofol pharmacokinetic-pharmacodynamic model in general anaesthesia

چکیده
BackgroundTarget-controlled infusion (TCI) systems incorporating pharmacokinetic (PK) or PK-pharmacodynamic (PK-PD) models can be used to facilitate drug administration. Existing were developed using data from select populations, the use of which is, strictly speaking, limited these populations. Recently a propofol PK-PD model was for broad population range. The aim study prospectively validate this in children, adults, older subjects, and obese adults undergoing general anaesthesia.MethodsThe 25 subjects included each four groups stratified by age weight. Subjects received through TCI with Eleveld model, titrated bispectral index (BIS) 40–60. Arterial blood samples collected at 5, 10, 20, 30, 40, 60 min after start infusion, every 30 thereafter, maximum 10 samples. BIS recorded continuously. Predictive performance assessed Varvel criteria.ResultsFor PK, showed bias < ±20% but greater (−27%) subjects. Precision <30% all groups. For PD, wobble <5 units precision close Anaesthetists able achieve intraoperative values 40–60 effect-site target concentrations about 85–140% age-adjusted Ce50.ConclusionsThe predictive arterial plasma predictions low (population) when clinical anaesthesia practice.

Pharmacokinetic-pharmacodynamic modeling of propofol in children.
BACKGROUND The aim of this study was to identify the best model to describe pharmacokinetics and pharmacodynamics in prepubertal children and therefore to calculate the corresponding pharmacodynamic parameters. In addition, and to confirm our method, a group of postpubertal subjects was also studied. METHODS Sixteen children (9.5 yr, range 6-12) and 13 adults (22 yr, range 13-35) were include...

متن کاملAnalysis of pharmacodynamic interaction of sevoflurane and propofol on Bispectral Index during general anaesthesia using a response surface model.
BACKGROUND Propofol and sevoflurane act on the GABA(A) receptor, modulating the function of this receptor in an additive manner. The pharmacodynamic interaction of both drugs considering their effect on EEG activity analysed by the bispectral index (BIS) was identified as additive, but this has not been studied in a clinical setting. The objective of this study was to analyse the pharmacodynami...
متن کاملEffectiveness, safety, and pharmacokinetic and pharmacodynamic characteristics of microemulsion propofol in patients undergoing elective surgery under total intravenous anaesthesia.
BACKGROUND The aims of this study were to investigate the effectiveness, safety, pharmacokinetics, and pharmacodynamics of microemulsion propofol, Aquafol (Daewon Pharmaceutical Co., Ltd, Seoul, Republic of Korea). METHODS In total, 288 patients were randomized to receive 1% Aquafol or 1% Diprivan (AstraZeneca, London, UK) (n=144, respectively). A 30 mg test dose of propofol was administered ...
متن کاملClinical Pharmacokinetic and Pharmacodynamic Profile of Rivaroxaban
Rivaroxaban is an oral, direct Factor Xa inhibitor that targets free and clot-bound Factor Xa and Factor Xa in the prothrombinase complex. It is absorbed rapidly, with maximum plasma concentrations being reached 2-4 h after tablet intake. Oral bioavailability is high (80-100 %) for the 10 mg tablet irrespective of food intake and for the 15 mg and 20 mg tablets when taken with food. Variability...
